‘Round the clock health assistance
We all know that if you have a life-threatening emergency, it’s important to call 9-1-1. If you have a sudden health problem that is NOT an emergency, the first person to call is your doctor. He or she will tell you what you should do.
If your doctor can’t see you, or if you're unable to reach your doctor’s office, contact Customer Service at (866) 420-6782, TTY: (877) 613-2075. From this number, you can reach our Nurse Advice Line 24 hours per day. Or if you call during Customer Service hours (Mon.- Fri., 8 a.m. to 5 p.m., excluding holidays), we can help arrange an appointment with another doctor or direct you to the urgent care center nearest you.
Taking charge of your health condition
Are you living with a chronic health condition? Harbor Health Plan offers Disease Management (DM) programs to help improve your health. The goal of DM programs is to provide education to you and/or your caregivers if you are diagnosed with one or more specific conditions. This information can help you to make behavioral changes and self-manage your condition(s). By making the right choices, you can improve your health and quality of life.
Why should you take advantage of DM programs? In addition to being available at no cost to you, they can help you to:
- Understand and follow your doctor’s treatment plan
- Manage your risk for developing new conditions
- Decrease complications from your existing condition(s)
- Reduce unnecessary hospital stays or emergency room visits
- Learn to manage your condition(s) better
- Improve your overall health.
Disease Management programs are available to you if you have been diagnosed with:
- Coronary Artery Disease (CAD)
- Borderline diabetes
- High cholesterol
- Hypertension (high blood pressure)
- Pregnancy/postnatal issues regarding maternal/child health

Choosing or changing your Primary Care Provider
Do you need to choose a Primary Care Provider (PCP) for the first time or to change to a new PCP? Call Customer Service or download the PCP Selection Form. Follow the instructions on the form to make your selection and send it to us.
Prior Authorization Notice
Some medical services and prescription drugs require a prior authorization before members can receive them. Please check with your Primary Care Provider, who will request prior authorizations for you. The medical services and prescription drugs that require prior authorization may change at the start of or during the plan year.
Third Party Entity Payments Notice
Harbor Choice does not accept premium and cost-sharing payments from third party entities made on behalf of enrollees with the exception of the Ryan White HIV/AIDS Program; Indian tribes, tribal organizations or urban Indian organizations; and state and federal government programs.
Health Risk Assessment
Harbor Choice believes in proactively reaching out to our members to learn about barriers to receiving health care that is right for them. You may be contacted by telephone to complete a Health Risk Assessment, also called an HRA. During this call, you will be asked questions about your health and health-related needs.
If you have not received a phone call, and would like to complete a HRA, please contact Harbor Choice Customer Service at (866) 420-6782, TTY: (877) 613-2075. We will mail an HRA upon request.
